Envolve's Disease Management programs are a system of coordinated healthcare interventions and communications created for those with conditions where self-care efforts, care gap resolution, and improved coordination of care can be implemented.
Envolve’s disease management programs empower individuals and their healthcare providers to manage their conditions and prevent exacerbations with the goal of improving health outcomes and reducing the costs of care, one person at a time.
As a result of our study on motivational theories, a Positive Psychology-based coach training curriculum was developed to approach disease management. The training, which all Health Coaches receive, focuses on application of key components of positive psychology and the effects of positive emotions in changing behaviors - with the goal to empower individuals to be active, informed participants in their health. Additionally, we evaluate each member’s readiness to change through a series of questions based on the Prochaska Transtheoretical Model of Change (James O. Prochaska, PhD, 1983). The model proposes that lifestyle change is a gradual evolution in thought process and action.
